                        FOOTBALL AGILITY & TECHNIQUE CAMP

Camps Philosophy: To help build athlete's confidence and provide them with multiple
opportunities to succeed and they will define their path to success!
The camp will focus on basic fundamentals of the agilities and techniques involved with the
game of youth football. The camp will operate in an organized and fun environment, while
emphasizing the importance of academics, teamwork and a strong work ethic. This camp will
be both demanding and fun, with an end result of improved football skills and the ability to
work as an effective member of a team.

                                                   (Monday, Tuesday, Wednesday & Thursday)
                               CAMP TIMES: 6:30 PM TO 8:00 PM @ N. FT. MYERS COMMUNITY PARK
                                       Youth ages: from 10 to 16 Experience levels: from youth to high school
                                                        Camp costs: $40.00 per participant
                                                    (Make checks payable to "In The Zone")
                                    (includes all training and equipment with water supplied at the field)

AGILITY & TECHNIQUE CAMP. I HEREBY GIVE MY APPROVAL TO THE PARTICIPATION IN ANY AND ALL OF THE

ACTIVITIES OF THE CAMP. INCLUDED BUT NOT LIMITED TO DRILLS, TRAINING INSTRUCTION AND ANY OTHER

ACTIVITY ASSOCIATED WITH THE CAMP ON AND AFTER THE DATE HEREOF. I FURTHERMORE HEREBY RELEASE,

ABSOLVE, INDEMNIFY AND AGREE TO HOLD HARMLESS THE ORGANIZERS, SUPERVISORS, COACHES, FACILITY,

AND THE VOLUNTEERS FROM ANY CLAIM, DEMAND, OR ACTION ARISING OUT OF, OR IN ANY WAY RELATED TO

THE CAMP OR RELATED ACTIVITIES, INCLUDING BUT NOT LIMITED TO ANY INJURY TO MY CHILD EXCEPT AND

ON TO THE EXTENT OF AND IN THE AMOUNT COVERED BY ANY ACCIDENT OR LIABILITY INSURANCE. IN THE

EVENT OF AN INJURY, "IN THE ZONE" FOOTBALL AGILITY & TECHNIQUE CAMP STAFF IS AUTHORIZED BY ME TO

OBTAIN ANY MEDICAL CARE OR TREATMENT DEEMED NECESSARY.

I HAVE CAREFULLY READ THE ABOVE WAIVER AND RELEASE AND FULLY UNDERSTAND THIS IS A RELEASE

OF LIABILITY AND I SIGN IT VOLUNTARILY.
